Both parents and students must create an account and link them together.  This will be the only way athletic paperwork will be taken. 

Physicals must be dated after April 1, 2024 in order to be valid.  Please use all online forms when filling out information.

Only two items that should be uploaded.
1.  Doctor’s Physical Exam dated after April 1, 2024 with doctor signature
2.  Birth Certificate
